For your convenience, we are providing forms that you may need for registration that you can fill out and turn in to our Parish Office. You may submit the form to the coordinator in these ways:
- Upload: Using the fillable forms, submit to our website.
- Email: you may also email your filled out form and scanned documents to the coordinator
- By Postal Mail: when you’re done filling gout the form, simply print it and mail it to the attention of the coordinator or department.
- At the Office: you may also drop off your filled out printed form and drop it off at the office together with other necessary documents and payment.

Formularios
Para su seguridad y conveniencia, le proporcionamos formularios que puede necesitar para registrarse. Puede enviar el formulario al coordinador de estas formas:
Correo electrónico: también puede enviar por correo electrónico el formulario completo y los documentos escaneados al coordinador
Correo postal: cuando haya terminado de llenar el formulario, simplemente imprímalo y envíelo por correo con atención al coordinador o departamento.
En la oficina: también puede dejar su formulario impreso completo y entregarlo en la oficina junto con otros documentos necesarios y el pago. Tenga en cuenta todas las pautas de seguridad y el horario de atención si decide seguir esta ruta.
